i did a lot of heavy work when i was a working man and this is how i sometime greet the doc;

 Doctor: How much pain are you in?
Me: The normal amount.
Doctor: The normal amount is zero
Me: Buddy I idle at at least a four.

sometimes it goes to 'oh, shit' way beyond the ten mark
